My boyfriend has a bad fetish?

My boyfriend has a fetish of being kicked in the balls. He asks me to do it for him mutiple times a day. The doctor said he has mutiple bruises on his balls, and for me to stop, but if I don't do it, he pays stranger girls to kick him in the balls as hard as they can, which I don't like. I've asked him to stop, and he said he would but he says it's an addiction that he can't control. Should I send him to an addiction clinic? I'm worried. Help!
